Appointments
If you are new to our practice, you can download our intake forms and bring them,
completed, to your first visit. Or, please arrive 10 minutes early and fill in new
patient forms before your appointment. If you need to cancel an appointment, call
the office 24 hours in advance to avoid a no-show charge. Fax all requests for records transfer or Disablity forms to 804-285-9805.

We make every effort to see you promptly at your appointment time. If there is a delay (emergency, delivery, etc.), we will inform you when we expect the doctor to return. If you cannot wait because of other commitments, we encourage you to reschedule your appointment. We welcome feedback or suggestions on how we can make your visit a positive experience.
Our Reception Area
In an effort to provide a pleasant environment for all patients, smoking, eating and drinking are not permitted in our reception room. Children are always welcome but we ask that you be aware of their actions and make sure the children do disturb other patients. A telephone is available at the check-out area, and you can call for insurance information or arrange for transportation.
Please turn off all cells phones when entering the office. This practice will ensure that you are aware when you are being called to the front for registration or when you nurse is calling you for your appointment.
24-Hour Phone Access
Our office telephone, (804) 282-9479, is answered by a live operator 24 hours a day. When the office is closed, please be prepared to inform our answering service about the urgency of your call so they can relay the correct information to the on-call doctor. At times, another obstetrician/gynecologist will be taking calls for the office and our answering service will direct you accordingly.
Confidentiality Respected
Your written authorization is required for the release of any information from your medical chart. When you first visit our office, you will complete an authorization form, listing everyone allowed to receive information from your chart. If any person, other than your insurance company, requests information and is not listed on the form, we will contact you for permission. Please promptly notify the office of any changes to your personal information.
Fees
We require that you pay all co-payments, deductibles and any non-covered services at the time they are rendered. We accept cash, money orders, checks, Visa, MasterCard, American Express and Discover. If you are a self-paying patient, please talk to the practice administrator to make payment arrangements.
Non Medical Fees
- Transfer of Medical Records - $15.00
- Processing of Disability Forms - $25.00
- Appointment No-Shows - $25.00
- Returned check : we pass along the charge we incur from the banking center
Billing Your Insurance
We will prepare and submit your insurance claims for all your obstetrical and gynecological care. Please bring the following items so the billing process will go smoothly:
- A copy of your current insurance card
- Insurance or Physician referral if required
- List of all medications and known allergies
- Recent test related to your visit
